I authorize Naturally Given Product LTD. to charge a monthly membership fee of $160.00 plus GST = $168 for twelve consecutive months to comply with a membership agreement that gives (__2__) session(s) each month for the next twelve months.

Sessions need to be used during the month paid and cannot be saved up,
transferred to another person, or used at a later time / month.

A cancellation fee of $80 plus GST will be charged to your credit card for each session remaining until the end of your membership agreement if you decide to cancel early.
